Description: This half-day meeting will focus on the importance of CHWs in the prevention and management of chronic diseases in Illinois. Sessions include the roles and goals for CHWs nationally and statewide; a history of the IL Community Health Worker Advisory Board; and a review of recommendations from the 2016 IL CHW Advisory Board Report, Community Health Workers in Illinois-A Value Driven Solution for Population Health. Coffee and light refreshments will be available.
