The University of Illinois at Chicago School of Public Health is sponsoring scholarships to attend the Public Health 3.0 Illinois Workforce Development Forum. Scholarships will be provided for registration and lodging.
Scholarships are limited to local health department attendees serving in jurisdiction under 115,000. Up to 20 scholarships are available to one representative per health department.
Please submit your application and registration information will be emailed to you.
Scholarships will be provided on a first come, first serve basis, please submit your applications no later than Friday April 13th.
