Funding Opportunity for Healthcare Organizations - Medication Assisted Recovery (MAT/MAR) Implementation
What: $10,000 Organizational Grants for Medication Assisted Recovery (MAR/MAT) Services
Who: FQHC’s, Clinics, Health Departments, Hospitals, etc.
Criteria: Health care organization that has or will begin implementing MAR services to their patients between August 2021 - April 2022.
How: Submit 1 year implementation plan and application via this form. A total of 10 Organizational Grants will be Awarded on a first come fist serve basis (apply today!)

Healthcare organizations must agree to meet the following requirements:
- Healthcare organizations must provide a 1 year plan for Medicated Assisted Recovery implementation. The 1 year plan must be uploaded at the bottom of the application linked above.
- Implementation must include at least one (1) provider's participation and completion of the X-Waiver Training between August 2021 and April 2022. Providers must complete a separate application. Providers will receive an additional $1,000 stipend for participation in training and $1,000 for first prescribing MAR/MAT medication between August 2021 – April 2022.
- Healthcare organizations must agree to submit at least one (1) provider to be mentored in the MAR/MAT mentorship program. Your mentor will be assigned to offer additional provider supports during implementation.
- Healthcare organizations must agree to serve as a spoke site for SIU SoM Opioid ECHO. This will require a case presentation from at least one (1) provider.
A webinar will be held in February to discuss starting a MAR/MAT program and address any questions on program.
