Food as Medicine is a program used to prevent, manage, or improve chronic diseases and well-being through food and nutrition interventions. Cook County Health (CCH), a safety-net health system, has a significant patient population facing food insecurity and piloted a Food as Medicine program for food-insecure patients with diet-related health conditions. The CCH Food as Medicine team continues to evaluate how programs like these can improve clinical health outcomes and social factors related to health.
