Community Health Workers (CHWs) play a vital role in improving health outcomes, advancing health equity, and connecting communities to care. As Medicare reimbursement opportunities for CHW services continue to expand, public health agencies, healthcare systems, and community-based organizations need practical guidance on implementation and billing processes. This session will provide an overview of current Medicare billing pathways for CHW services, including eligible activities, documentation requirements, workflow considerations, and strategies for program sustainability. Participants will gain practical insights into how organizations can integrate reimbursable CHW services into clinical and community health settings and lessons learned from two organizations who have billed Medicare for services in Illinois.
