This training will highlight the role of chaplaincy in addressing health care and social determinants of health (SDoH) needs. There will be a focus on the MCH ACEs experience of caregiving for a child with special physical, neurodevelopmental, mental health, and/or acute and chronic illness. You will hear stories of lived experience from caregivers about how they relied on spiritual/religious support. You will learn how Advocate Aurora Health developed the first evidence-based model, the Patient-Centered Spiritual Care Model, for integrating chaplaincy into clinical care planning and for a closed-loop design to address health care and SDoH needs. This model considers patient-centered beliefs, recognition of cultural values, and emotional support when implementing the intervention. You will gain actionable skills and tools to support interventions and execute resource and referral activities.
