This second part of the series will cover how CHWs can be public benefits liaisons between community members and case managers at the Illinois Department of Human Services and Illinois Department of Healthcare & Family Services. Tips and tricks will be given on when, how, and what to communicate with these case managers. This session will also have a mini panel featuring CHWs and other professionals at HFS, IPHCA, VNA, and IPHA. We will review case studies, discuss real-life scenarios, and detail how a signed letter from a CHW.
