Community Health Worker (CHW) Strategy Meetings (March 2019)

 

Description:  This half-day meeting focused on the importance of CHWs in the prevention and management of chronic diseases in Illinois. Sessions included the roles and goals for CHWs nationally and statewide; a history of the IL Community Health Worker Advisory Board; and a review of recommendations from the 2016 IL CHW Advisory Board Report, Community Health Workers in Illinois-A Value Driven Solution for Population Health.

 

Objectives:  At the end of the meeting, the participant were be able to:

  1. Understand the current and future goals toward implementing and integrating CHWs in Illinois.
  2. Identify and discuss gaps and areas for development in the 2016 CHW Advisory Board Report, Community Health Workers in Illinois-A Value Driven Solution for Population Health.
  3. Inform frequency, location and topic areas that need to be addressed during future IL CHW workgroup and advisory meetings.
